Hillingdon Slipstreamers Go-Ride MTB Cross Country Racing

Important Notes

• A cycling helmet MUST be worn at all time during the Go-Ride activities.
• Any riders who persistently misbehave or put others in danger will be asked to leave the event.
• It is the parent’s/guardian’s responsibility to ensure that his/her child’s bike is in a safe condition to ride.
  
• Go-Ride cycling and racing is meant to fun - let's keep it that way!
  

Parental/ Guardian Declaration and Consent to Participate

• By submitting this application, I have read the information provided to me by the organisers, and hereby consent to my child taking part in the Go-Ride races on the 14th November 2015.
  
• I understand and agree that my son/daughter participates in the sessions entirely at his/her own risk. 
• I understand that the safety of my son/daughter's bike and equipment is my responsiblity and not that of the
  British Cycling Go-Ride Coaches/ Volunteers.
• I have considered the nature of these activities and have discussed them with my son/daughter, and I am satisfied that my son/daughter is sufficiently responsible and competent to assume responsibility for his/her own safety under the supervision of the British Cycling Go-Ride Coaches/ Volunteers.
  
• I am aware that photographs/video footage may be taken and give permission for them to be used as appropriate – I will notify the Go-Ride team if I do not.
  
• I confirm that I will provide details of any relevant medical conditions that may affect my child taking part in the Go-Ride activity to the organisers on the day of the event. If I do not supply this information to the organisers, it is safe to assume that my child does not suffer from any medical condition/s.
