Consent

I give permission for my child named in the booking form to participate in the inflatable activities provided at the PTA Summer Social event.

I understand that participation in inflatable activities such as bouncy castles, inflatable slides and obstacle courses involves physical activity and carries inherent risks, including slips, trips, falls, collisions and other accidental injuries.

I confirm that my child is medically fit to participate and that I have informed the organisers of any relevant medical conditions, allergies or additional support needs.

Parent/Guardian Responsibility and Supervision

I understand and agree that I remain responsible for the supervision, behaviour, welfare and safety of my child at all times during the event.

I acknowledge that:

• The PTA, school, volunteers and inflatable operator are not providing child-care services.
• A parent, guardian or responsible adult nominated by me must remain on site throughout the event.
• I am responsible for ensuring my child follows all safety instructions, signage and directions given by event organisers and inflatable operators.
• Children who do not follow safety rules may be asked to leave an activity.
• Participation in the event does not transfer responsibility for my child to the PTA, school, volunteers or inflatable operator.

Medical Treatment

In the event of an accident or medical emergency, and if I cannot be contacted immediately, I authorise the organisers to seek appropriate medical assistance for my child.

Declaration

I have read and understood this form. I give permission for my child to participate in the inflatable activities and acknowledge my responsibility for supervising my child throughout the event.

Important: Parents/guardians remain responsible for their children at all times while attending this event.
